Health Declaration and Risk Acknowledgement

This declaration applies to every person who exercises, trains, receives instruction or otherwise participates in physical activity at Hound Performance Studio, operated by HOUND PERFORMANCE LTD.

This document is not a substitute for medical advice. It does not exclude or limit any liability that cannot lawfully be excluded.

1. Nature of physical training

Strength training, conditioning, mobility work, cardiovascular exercise and the use of gym equipment involve inherent risks. These risks can arise even when equipment is maintained and used correctly.

Risks may include muscle soreness, strains, sprains, joint injury, falls, impact injury, trapped fingers, dropped weights, fainting, dizziness, shortness of breath, abnormal blood pressure response, aggravation of an existing condition and, in rare cases, serious injury or death.

Obtain advice from a doctor or other appropriately qualified healthcare professional before using the Studio if you have, suspect you may have, or have recently experienced any condition that could be affected by exercise, including:

4. Duty to keep your health position under review

Your health may change after joining. You must reassess your fitness before each visit. If your health, medication, pregnancy status, injury or symptoms change, obtain appropriate advice before continuing where the change could affect safe participation.

You are not required to provide detailed medical records to the Company. If you choose to disclose health information, the Company will handle it in accordance with the Privacy Policy.

5. Unmanned and independent use

You understand that the Studio may be unmanned and that staff or first aid personnel may not be present.

You are responsible for exercise selection, loading, intensity, technique and equipment use. You must not attempt exercises or loads that you cannot control safely.

Use safety equipment and a suitable spotter where appropriate. Do not rely on CCTV as a means of supervision or emergency monitoring.

6. Trainers, coaches and therapists

An independent trainer, coach or therapist is responsible for their own professional advice, screening, programming, supervision and services.

You remain responsible for informing the professional of any relevant limitation or change in your condition. The Company is not responsible for independent professional services except where the law makes it responsible.

7. Symptoms requiring immediate action

Stop exercising immediately if you experience chest pain, severe or unusual shortness of breath, faintness, loss of coordination, confusion, sudden weakness, severe pain, a suspected fracture, a head injury, an allergic reaction or another symptom that may require urgent care.

Call 999 in an emergency. Do not continue training while waiting to see whether a serious symptom improves.

8. Accidents, incidents and unsafe equipment

Report accidents, injuries, near misses, equipment faults and hazards to the Company as soon as possible. If equipment appears unsafe, stop using it and prevent others from using it where you can do so safely.

9. Assumption of inherent risk

You voluntarily accept the ordinary and inherent risks of physical exercise and gym equipment use, including risks that remain after reasonable safety measures have been taken.

This acknowledgement does not excuse negligence by the Company and does not remove any statutory right or remedy.

10. Persons under 18

A person under 18 must not attend alone. A parent, legal guardian or supervising adult must ensure that the activity is suitable for the young person's age, maturity, health, experience and ability.

The Company may require written parental or guardian consent and may restrict particular exercises or equipment.

11. Confirmation

By accepting this declaration, you confirm that you have read and understood it, that the information you provide is accurate to the best of your knowledge, and that you will use the Studio responsibly.
